To provide competent and comprehensive healthcare in an ambulatory setting to all who seek good health.

Patients with questions concerning their care are welcome to call the practice during normal working hours. After regular hours a physician is on call for emergency situations with current patients.  Please give your pharmacy three days notice to allow sufficient time for our office to refill your medications.  We are not able to refill antibiotics, narcotics, or routine medications after normal office hours or on weekends, so please plan accordingly.

In case of Emergency - Call 911 or go to the nearest emergency department for further evaluation and treatment. 

• We are concerned with the high costs of medical care and make every effort to keep costs down. With that in mind, we require payment in full for any balance due at the time medical services are rendered.
• If you have difficulty paying the day of your visit please discuss your situation before your appointment is made.
• All co-pays are collected when you check in for your appointment.
• A fee of $20 may be charged for completion of forms.
• Returned checks are subject to a $35 charge and require that all future payments be made in cash.
• For your convenience we accept cash, check, VISA, MasterCard, traveler's checks, and debit cards.
• If you are not current with a payment plan for your account, we will be unable to schedule non-emergency appointments.

• Patients are seen by appointment only. If you believe you will be late for your appointment, please call the office as soon as possible.  You may be able to have your appointment rescheduled for later the same day or be worked in between our other scheduled patients.  Appointments must be cancelled or rescheduled within 24 hours to avoid a No Show Charge of $50.  Same Day Appointments may also be charged a No Show Charge if not cancelled or rescheduled timely to allow for rebooking.  We do charge a $50 missed appointment fee for any appointment that is missed or cancelled less than 24 hours ahead of time.
